An off-duty Detective Marv Bozwick pursues a criminal through the city sewers on July 4th. After being attacked from behind by a man in a pig mask, Bozwick wakes up in a subway tunnel, hanging by his tongue, and is given the option of tearing out his tongue to save his life or waiting for the next train to come, which would kill him. As a result, Bozwick was fatally struck by the train. Police Captain Angie Garza sends Zeke Banks a new partner, idealistic rookie William Schenk, to work with him the next day. After investigating Bozwick's murder with Schenk and Banks, Banks finds that Bozwick's killing follows a similar pattern to that of the Jigsaw Killer.

Meanwhile, a murder investigator called Fitch is kidnapped and put in a trap where he must pull his fingers off to prevent electrocution in a filling water basin; he too fails to escape and dies. Fitch had rejected a backup call from Banks some years earlier, almost killing him. Due of his relationship with Fitch, several police accuse Banks of being the perpetrator. The station then receives a parcel containing a pig puppet and a bit of Schenk's tattooed flesh. The police are directed to a butcher shop by a little vial within the box, which was once a hobby store where Banks and his father, retired chief Marcus Banks, would frequent. The squad finds a recording recorder and a skinned body identified as Schenk when they arrive. Marcus decides to seek out the murderer himself and visits a warehouse, where he is kidnapped. Garza is abducted and put in a trap in the cold storage of the precinct, where she must slice her spinal chord with a blade to prevent hot wax from running through a conduit over her face. She fails to do so, and when Banks finds her corpse, she dies from her injuries caused by the boiling hot wax.

Emmerson gives Banks a last test, showing him Marcus suspended in the air and steadily losing blood. Emmerson phones 9-1-1 and says he's being chased by a gunman, so a SWAT squad is sent to his area. He gives Banks a handgun with one cartridge and gives him the option to rescue Marcus but let Emmerson escape or murder Emmerson and let Marcus bleed to death. Banks shoots the target to free his father, then fights Emmerson. The SWAT squad enters and accidentally trips a tripwire, yanking Marcus' shackles again. Marcus' arm moves, revealing a gun, so SWAT kills him thinking he's the shooter. Emmerson flees as Banks cries.

Following Jigsaw's release, Stolberg and Goldfinger pitched a new Saw film focused solely on John Kramer / Jigsaw rather than any of his established apprentices to series veterans Mark Burg and Oren Koules, but Burg and Koules called the duo to inform them about Rock's ideas for a new film, with Rock contacting them shortly afterwards to discuss his concept. Rock assisted Stolberg and Goldfinger with their writing, reworking the tale as needed.

Stolberg and Goldfinger once proposed an after-credits scene in which Kramer met a young Schenk after the murder of the latter's father and became friends with him, maybe giving him the puppet he later uses as the Spiral (2021) Killer.

The film began pre-production on May 16, 2019. Darren Lynn Bousman, a former series director, will direct the picture, with Burg and Koules serving as producers. Rock served as executive producer and wrote the narrative treatment.
